Do You Really Need Preimplantation Genetic Diagnosis?

do-you-really-need-preimplantation-genetic-diagnosis
Preimplantation genetic diagnosis helps detect embryo abnormalities early, guiding IVF decisions and reducing the risk of inherited disorders.

What Is Preimplantation Genetic Diagnosis (PGD)?

Preimplantation Genetic Diagnosis (PGD or PIGD) is a specialized genetic test performed on embryos before they are implanted in the uterus. In some cases, it may also be used to screen eggs before fertilization. PGD is commonly used during in-vitro fertilization (IVF) to detect genetic abnormalities, helping ensure that only healthy embryos are selected for transfer.

PGD is different from Preimplantation Genetic Screening (PGS). While PGS is done even when both parents are genetically normal (to check for chromosomal abnormalities), PGD is specifically recommended when one or both parents carry a known genetic defect that could be passed on to the child.

How Is PGD Done?

PGD is performed as part of the IVF process. It involves several steps:

  1. Ovarian Stimulation
The IVF cycle begins with hormone injections to stimulate the ovaries and produce multiple mature eggs. These eggs are collected during ovulation.
  2. Sperm Collection
A sperm sample is obtained on the same day. If collection is difficult or if conditions like azoospermia are present, sperm may be retrieved through specialized techniques.

  3. Fertilization
Each egg is placed in a laboratory dish with thousands of sperm cells to allow fertilization.
  4. Embryo Development
The fertilized embryos are allowed to grow for about five days until they reach the blastocyst stage.
  5. Embryo Biopsy
A few cells are gently removed from each embryo, and the embryos are then frozen for safety.
  6. Genetic Testing
The DNA from the biopsied cells is tested for specific genetic disorders. This process typically takes more than a week.
  7. Embryo Transfer
Only embryos free of genetic abnormalities are selected and transferred to the uterus. Healthy embryos that are not used immediately are frozen for future cycles. Embryos with confirmed genetic defects are usually discarded.

Who Should Consider PGD?

PGD is recommended for couples who are at higher risk of passing genetic conditions to their children, including:

  • Carriers of sex-linked genetic disorders
  • Couples with a known single-gene disorder
  • One or both parents with chromosomal abnormalities
  • Women 35 years or older
  • Couples with repeated IVF failures
  • Women with a history of recurrent pregnancy loss

Benefits of PGD

Preimplantation Genetic Diagnosis offers several advantages:

  • Screens for hundreds of genetic disorders, reducing the risk of inherited conditions.
  • Helps couples make well-informed reproductive decisions with clarity and confidence.
  • Ensures the healthiest embryos are selected, reducing pregnancy-related risks.
  • Significantly lowers the chances of passing on serious genetic diseases.

While PGD cannot detect every genetic abnormality, it greatly reduces the likelihood of many serious disorders.
